Travel Nurse Documentation

  • Copies (not originals) of all active professional licenses for those states that are not yet paperless. Make sure to photocopy both the front and back. 

  • Titer results with lab values for the following:

    • Rubeola

    • Rubella

    • Varicella

    • Mumps

    • Hep C and Hep B titers may also be required.

  • Proof of a Tdap vaccine.

  • Documentation of an annual seasonal influenza vaccination.

  • COVID-19 PCR or Antigen Test and/or COVID-19 Vaccination.

  • COVID-19 vaccine.

  • Documentation including implantation date, read date, and mm of your last two or three PPDs.

  • Basic Life Support (BLS), ACLS and PALS certifications issued by the American Heart Association (AHA). ACLS and PALS will be required as determined by your specialty. All certifications must be signed.

  • A physician statement dated within the 12 months prior to your start date.

  • Respirator fit test. Whenever you have a respirator fit test, request a copy of the results for your records; you will need it.

  • Legible copy of your driver’s license, preferably in color.

  • An official copy of your high school transcripts and/or high school diploma.

  • An official copy of your college transcripts specifying degree earned and date of degree and/or your college diploma.

Every facility has different requirements, and they change frequently, so be sure to ask your Recruiter about the certifications that are most important for the positions in which you’re interested.
